Release of Information (ROI) Forms

A Release of Information form allows us to share your information with other healthcare providers, school personnel, or other parties you specify. You can request specific information to be released and revoke authorization at any time by contacting Tari Miller, Clinical Director, at 201-248-7395 or tarimiller@beautifuljc.com.

Medical Records Requests

To request your medical records, fill out our Records Release form specifying the information you need. You can submit this form in person or contact Tari Miller, Clinical Director, at 201-248-7395 or tarimiller@beautifuljc.com. There may be a nominal processing fee depending on the extent of records requested.

Can I specify what information is released?

Yes, you can request specific documents, dates of service, or types of information to be included or excluded when releasing your records.

Your Privacy is Protected

The law protects the relationship between client and counselor. Information cannot be disclosed without your written permission, and we follow strict HIPAA protocols to keep your information secure.

Exceptions to Confidentiality

There are limited situations where we are legally required to breach confidentiality:

  • Suspected child, dependent adult, or elder abuse

  • If a client is threatening serious bodily harm to another person

  • If a client intends to harm themselves and will not cooperate with safety planning

  • If a therapist is subpoenaed along with records

We take your privacy seriously and will always explain our legal and ethical obligations clearly.

Cancellation Policy

24-Hour Notice Required

Please email or call if you cannot keep your scheduled appointment. We require at least 24 hours notice to avoid being charged for your missed session.

Life happens—we understand. Just let us know as soon as possible so we can offer your time slot to someone else who needs support.
